On Saturday, January 16, 2016 at 5:18 PM, the Purchase and West Harrison Fire Departments and Harrison EMS were dispatched to Interstate 684 southbound on the ramp to Interstate 287 eastbound for a reported tractor-trailer rollover with entrapment. Car 2415 arrived on scene, confirming that extrication was required. Upon the arrival of Engine 240, firefighters put its extrication tools into operation, removing the driver's door to allow patient removal. The driver was transported to Westchester Medical Center by 61-A1.

Crews examined the tractor's side-mounted saddle tanks for failure and awaited the arrival of heavy wreckers to upright the rig. One 1-¾" foam line was deployed as a precaution. Purchase units remained on location for five hours until Mark's Towing removed the tractor and trailer.
